Description of Duties: The Blended Case Manager (BCM) will promote resiliency and recovery for children, adolescents, their families and adults by accessing, coordinating and mentoring community resources and services.
Qualifications: A blended case management staff person shall meet one of the following criteria:
A Bachelor’s degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education
Requirements:
~ Proof of education
~Act 33/34 clearances newer than 12 months
~FBI clearance
~Valid Pa drivers license
~ Reliable transportation
~Proof of Pa insurance
~Physical health examination- within one year
~Effectively manage a case load of 25-30
~Share the responsibility of 24/7 on-call coverage with the other case managers
~Attendance at weekly staffing and supervision meetings
~Participation in annual service trainings
